Subscription Agreement
1. On page 1, insert the effective date of the subscription, check off the Class being subscribed to and insert the subscription amount.
2. On pages 2-3, complete the required investor information. Under Part 3, please check off how the subscriber qualifies as an accredited investor.
3. On page 3, the subscriber (and joint subscriber, if applicable) must sign to complete the agreement.
4. For Individuals only, on pages 4-5, the subscriber must complete and sign Form 45-106F9. Check off the applicable funds in section 1, initial in sections 2 and 3, and sign in section 4. 

Registered Accounts

Subscription Agreement
1. On page 1, insert the effective date of the subscription, check off the Class being subscribed to and insert the subscription amount.
2. On pages 2-3, complete the required investor information. Under Part 3, please check off how the subscriber qualifies as an accredited investor.
3. On page 3, the subscriber (and joint subscriber, if applicable) must sign to complete the agreement.
4. On pages 4-5, the subscriber must complete and sign Form 45-106F9. Check off the applicable funds in section 1, initial in sections 2 and 3, and sign in section 4.

If you are subscribing through a registered account, please complete the RRSP application form (for RRSP subscriptions), the RRSP Direct Transfer form (if transferring funds from an existing RRSP account), or the TFSA application form (for TFSA subscriptions):

RRSP Form

RRSP Application (required for RRSP investments)
1. On page 1, check either the RRSP Member Plan or RRSP Spousal Plan box in upper left hand corner.
2. Complete Section 1 (Annuitant Information). If investment will be made under your spouse’s plan, also complete Section 2 (Spousal Account Information).
3. Skip Section 3 (Locked-In Supplement).
4. If there is a beneficiary to this plan, complete Section 4 (Designation of Beneficiary).
5. Skip Section 5 (Trustee Fee Authorization).
6. Complete and sign Section 6 (RSP Annuitant Signature).

Direct Transfer T2033 Form

RRSP Direct Transfer (required if transferring from an existing RRSP account)
1. Complete Area 1 (Annuitant Information) Parts A and B. Skip Part C.
2. Sign under Annuitant’s signature at the end of Area 1.
3. Return a signed copy of page 1 only.

 

TFSA FormTFSA Application (required for TFSA investments)
1. Complete Section 1 (Member Information).
2. If there is a beneficiary, complete Section 2 (Designation of Beneficiary).
3. Skip Section 3 (Trustee Fee Authorization).
4. Complete and sign Section 4 (RSP Annuitant Signature).

Subscribing as an Individual

A third-party is required to attest to your identity if you are not subscribing in the presence of a representative from Hamilton Capital. 

Subscribing as an Entity

Subscription Agreement Supporting Documents
Please attach two supporting documents to your subscription agreement, including one document from each of the following:

Document 1: Articles of incorporation; certificate of incumbency; the by-laws of the corporation; trust indenture; or a resolution of the Board of Directors that states the officers authorized to sign on behalf of the corporation.

Document 2: Certificate of corporate status; financial statements signed by an independent audit firm; or a notice of assessment for the corporation from a municipal, provincial, territorial or federal government.

Attestation of Identity Form (if applicable)
A third-party is required to attest to your identity if you are not subscribing in the presence of a representative from Hamilton Capital. 

Identification

Please attach a copy of government issued identification (passport or driver’s license) for the subscriber or, in the case of corporation or other entity, the authorized representative for the subscriber. 

Payment

Payment may be sent by cheque (payable to either “CTC ITF HCP Global Financials Market Neutral Fund” or “CTC ITF Hamilton Capital Global Financials Fund LP” or “CTC ITF HCP European Bank Fund LP”) or wire transfer. Are you an accredited investor?

Products offered by Hamilton Capital Partners Inc. have a minimum subscription amount of $150,000, unless you meet the definition of an "accredited investor" under National Instrument 45-106 Prospectus and Registration Exemptions.

If you meet the definition of an "accredited investor", you may invest a minimum of $25,000. Please consult the offering documents to determine your qualification status. Investment Advisors should consult their company's internal policies.

The Subscriber must be (and will be at the time the Subscription is executed) an accredited investor under National Instrument 45-106 Prospectus and Registration Exemptions ("NI 45-106")
